The scribe

Documentation you can trust, by design

Trust is earned in the details: consent before capture, resilience when the network drops, and a note that tells you when it found very little.

Consent-first capture

Clinicians read a standard consent script and record how consent was given: verbal, written or standing. Nothing is recorded, and no transcript is accepted, until consent is logged.

Resilient to network drops

Audio is also kept on the device. Anything live transcription misses during an outage is recovered from that audio before the note is drafted.

Only what was said

Every fact must be found, word for word, in the transcript. Anything that can't be found is discarded, and sections are never padded to look complete.

Evidence, not inference

Who said each fact, and when, is taken from the transcript itself. It is never guessed by a language model.

Honest about gaps

When a section has nothing behind it, the review screen says so, and the exported PDF records it too.

Smart glasses

The patient's record, hands-free

On ward rounds and at the bedside, a clinician's hands are often busy or gloved. With AurViz smart glasses and a phone in the pocket, they ask about the patient and hear the answer from the record, with its source.

    The glasses camera reads the QR code on the patient's wristband.

  2. Confirm it's you

    Fingerprint, face or PIN, through the phone's secure system prompt. Without a screen lock, no record opens.

  3. Ask naturally

    Speech is recognised on the device, and follow-ups like “and before that?” keep the thread.

  4. Hear the answer

    Spoken through the glasses and shown on the phone, with the source document and date.

Checked before it's spoken

In field testing, the assistant's language model added a time that the patient's record didn't contain. The answer was stopped before it reached the clinician, and the record was read out instead. Every model answer is checked against the record, line by line, before it is spoken.

Security and safety

Safeguards that don't depend on the model

Every AI model can be wrong. Medicaal's safeguards decide what reaches a signature or a clinician's ear.

Server-side signing control

The device only predicts; the server enforces. A note can't be signed while any unverified sentence carries a number, a drug or an allergy.

No clinical advice

Requests for advice are declined on the device before any network call, including those phrased as commands.

Biometric access

Records open only after the device's fingerprint, face or PIN check, and locking clears them from the device.

No sensitive logging

Logs record event types and sizes only, never a transcript, a question or an answer.

Append-only audit trail

Creation, consent, transcript, viewing, signing and export are each recorded with user and time, and no clinical content.

Models never exposed

AI models are bound to the server itself and are never reachable on the network. Devices talk to one Medicaal server at one pinned address.

FAQs

Is Medicaal cleared for clinical use?

Not yet. Medicaal is in pilot and is not a medical device. It has been tested end to end on real devices with demonstration data, and we're looking for clinical partners for the next stage of evaluation.

How accurate are the notes?

Every fact in a Medicaal note is traced to the words that were spoken, and unsupported doses, drugs and allergies can't be signed. Extraction is still being refined in pilot, so every note is reviewed by the clinician before it is signed.

What data has Medicaal been tested on?

Synthetic and demonstration data only, never real patients. Vital signs and lab values in our evaluation consultations come from Genkidata, an open, synthetic openEHR dataset published by the Berlin Institute of Health. Smart-glasses field testing used demonstration patient records.

Where does patient data go?

Consultation audio is sent to a cloud speech-recognition service for transcription. Everything after that stays on your Medicaal server: facts, notes, verification, signatures, PDFs and the audit trail.

The smart-glasses assistant sends questions only to your server.

Does Medicaal suggest diagnoses or treatments?

No. Medicaal documents what was said and adds nothing. The smart-glasses assistant declines requests for advice on the device, and they are never sent.

Can clinicians edit the note?

Yes, every section. Edits are re-verified against the consultation. An unsupported number, drug or allergy blocks signing until it's corrected, and other unsupported text must be explicitly accepted. Editing a signed note creates a new version that must be signed again.

What happens if the network drops mid-consultation?

Recording continues. Audio is kept on the device, and any missed stretch is transcribed from it before the note is drafted. Anything that can't be recovered is flagged on the review screen.

What hardware do we need?

An Android phone (Android 8 or later, or Android 13 or later for the smart-glasses voice assistant), a server to host Medicaal on your premises, and AurViz smart glasses for hands-free record access.
